Grove Wellbeing Centre provides a wide range of services for older people.

Professional Social Work Service

Our social workers help support and protect older people who may be at risk of physical, emotional or financial abuse or neglect.

They also work directly with highly dependent older people, for example, those with dementia, and provide professional support to carers.

Care Management

The care manager is responsible for completing a multi-disciplinary assessment of older people with more complex needs.

This can include those who want to continue living at home or those who need residential or nursing home care.

Intensive Domiciliary Support Scheme

This scheme is a multi-disciplinary service which provides care to older people living at home and who need constant assistance with day-to-day activities.

Any health or social care professional involved in caring for an older person can make a referral to the scheme, including doctors and hospital staff.

Older people and their families or carers can also contact the team to discuss their needs or arrange for an assessment.

Once the referral has been received, we can arrange a home visit for you and the views of the older person and their carers can be included in the assessment.
