I want to join the surgery. What do I need to do?

Joining our practice is quick and easy.

First, you need to make an appointment with our reception staff and explain that you are a new patient.

You can call them directly on 028 9016 4600.

You will be given an appointment and will need to bring your National Health Service (NHS) medical card with you.

If you have never held an NHS medical card, you will need to fill in a HS22x form, available from our reception staff.

This must be completed so you can receive a medical card.

During the appointment, you will be examined by a nurse or doctor and a brief medical will be carried out.

Once your registration is complete, you are free to make an appointment with any of our doctors.
