District nurses



District nurses are skilled professionals who provide co-ordinated care to patients in their own homes.

Although district nurses usually operate independently, they also work alongside other healthcare professionals, including GPs, to provide holistic, high-quality care to patients of all ages with acute and chronic illnesses.

The nurses can provide access to specialist nursing equipment and a range of teams dealing with continence, tissue viability, oncology and palliative care, diabetes, 24-hour nursing and heart failure.

Making an appointment

To see a district nurse, you must refer yourself or be referred by a doctor, family member or other healthcare professional.

Once the referral has been received, we can arrange an appointment for you.

All referrals should be made to the Central Referral Service, which is operated by the Belfast Health and Social Care Trust.

It is open at the following times:
  • Monday to Thursday - 8.30am to 5pm
  • Friday - 8.30am to 4.30pm.
